Please arrive 15 minutes
early for office visits. Should you need to reschedule,
please call at least 24 hours in advance.
Billing and Payment Policies
- Please bring your insurance card with you at the
time of service.
- We will bill your insurance for you if you provide
us with complete information.
- You are responsible for co-pays, deductibles, co-insurance
and noncovered or non-authorized
services.
- If your insurance company does not pay the full
amount due, you will receive a
statement showing an amount
due.
- All unpaid balances are reviewed for possible collections
after 60 days.
- For your convenience, we accept Visa, MasterCard
and Discover.
- We also offer budget payment plans to qualified
patients.

In
order to provide the best test and evaluation of
your sleep, your cooperation is essential. Please
follow
the instructions below. If you have any questions,
please call us at 269.324.0799 Monday through Friday
from
7 a.m. to 5 p.m.
Cancellation Policy
You must cancel a study at least 24 hours in advance,
or you may be charged $150.
Polysomnogram
(Night Sleep Study)
- Distractions, such as television, radios, cell
phones, pagers, etc., will need to remain off through
the night.
- Please feel free to bring comfort items such as
a pillow or blanket, but this is not necessary.
- Please bring sleeping attire. Women should consider
bringing two-piece attire to maintain
modesty and
to allow for ease of placement of monitoring
equipment.
- Please shampoo your hair prior to coming for the
test. Make sure it is dry and do
not use any hair
oil or hair styling products.
- Men should shave facial hair stubble; please do
not shave off a beard or mustache
just
for the sleep
study.
- Please do not use makeup or moisturizing lotion/oils
on face or body. These
items make it difficult to
apply some of our equipment.
- Try to maintain your usual daytime schedule. Avoid
any unusual physical
exercise that
you normally would
not do or any large meals in the
evening.
- Please don't take naps if at all possible on the
day of your study.
- This is an outpatient procedure. Nursing services
and medication
are not provided.
- Take your regular medication and if needed bring
it with
you and take
it
on schedule
unless otherwise
instructed.
- Studies
are concluded between 6 and 7 a.m., and the patient normally
leaves a short time after the study is concluded.

(Day Sleep Study)

Multiple Sleep Latency Test / Maintenance of Wakefulness
Test
(MSLT) / (MWT)
- Please shampoo your hair prior to coming for the
test. Make sure it is dry and do not use
any hair oil
or hair styling products.
- Men should shave facial hair stubble; please do
not shave off a beard or mustache just
for the study.
- Please do not use makeup or moisturizing lotion/oils
on your face. These items make
it difficult to apply
some of our equipment.
- Do not drink coffee or beverages containing caffeine
for 24 hours before the study.
- This is an outpatient procedure. Nursing services
and medication are not provided.
- Lunch will be provided for you. If you are scheduled
for a night study
prior to
the MSLT,
breakfast will
also be provided for you.
- Take your regular medication and if needed, bring
it with you and
take it
on schedule
unless otherwise
instructed.
- You may bring a book, magazines, needle work, laptop
computer, etc.,
with you.
